Caller alleged that there are bugs in the facility. Called slleged that the oven is so dirty that the fire alarm goes off every time it is used.
Based on observation, the AHS determined that there were roaches in the dining room and that the oven needed cleaning.

The caller alleged that the residents are not getting enough to eat. They are frequently served only grits for breakfast and a hot dog or bologna sandwich for lunch.
Bases on observation, record review and interviews, the AHS determined that the residents are not being served three nutritionally balanced meals per day. Substitutions to the menu are not being recorded, and one resident was not receiving a therapeutic diet as ordered.
