A resident, known to the facility as being a wanderer, was unsupervised and wandered across a busy 4 lane highway to a neighboring high school parking lot about 4:00 AM on June 27, 2007. He was found by two deputy sheriffs and returned to the facility. |
It was determined by observations, interviews, and review of resident and facility records that the facility failed to provide supervision in accordance with each resident's assessed needs, care plan , and current symptoms. |

Complainant indicated that the facility did not have towels, either paper or cloth for residents to dry their hands. |
On 10/27/09, DSS inspected 8 resident rooms. 8 of 8 did not have paper towels and or receptacles for the storage and immediate use of paper towels. The residents in these rooms indicated that they generally have to buy their own towels or ask staff to bring them towels from the home’s common bathrooms. |
