A resident's mattress is dirty and the bed unfit for use. |
The resident's mattress was observed to be sagging with numerous stains .The bedsprings were completely collapsed on the right side resulting in a downward slanted sleeping surface. |

5 of 7 residents interviewed stated snacks were not consistently offered 3 times per day. Facility menus did not list snacks. The snack list posted in the main dining area only indicated beverages offered at 10am and 3pm. when both food and beverages are required to be offered. |

Personal Care and Supervision |
A resident developed skin breakdown and infection after sitting in urine for extended periods. |
The resident was diagnosed with cutaneous candidiasis, an infection in which the key factor is prolonged exposure of skin to moisture.Other medical notes show the irritation and skin breakdown was most likely caused from extended periods of time sitting in wet disposable briefs. |

Residents were left at the facility with no qualified supervisor in charge . |
On 4-4-08 the Administrator in Charge left the facility to take a resident to a doctor appointment and failed to arrange a qualified Relief Administrator in Charge to stay with residents during that time. |
