On 6-04-08 and 6-09-08, a caller alledged that residents of the home had to wait outside in hot tempertaures for 30 minutes or more becuase staff was not available in the home. |
2 of 3 residents reported that on both 6-04-08 and 6-09-08, they had to wait outside for staff return to the facility, with temperatures being 91 and 97 on both days. |

At the 12-9-08 follow up visit a review of 2 of 2 residents' charts revealed medications had not been administered as ordered.An order signed 12-7-08 decreased dosages for two medications which were not relected on the Medication Administration Record(MAR) causing Resident A to receive more medication than ordered. Another Resident was not receiving 2 medications ordered: one because it had not been reordered until the 12-9-08 discovery and the other because the medication had not yet been located in one of three bags of medication Resident B brought with him at readmission on 12-2-08. |

On 11-5-08 a follow up monitoring visit revealed the facility was not in compliance with its Plan of Correction. A review of the November 2008 Medication Administration Record(MAR) revealed the dosage for Resident B's medication had been reduced by the Administrator without a doctor's order because the Administrator felt 3 tablets twice daily was too much.Resident B received one-third of the ordered dose. |

The facilty failed to assure accuracy of the MAR.Three medications from an old order had not been deleted from the October MAR,a medication to be administered Monday, Wednesday and Friday had been administered Wednesday, Thursday and Friday. Medication bottle labels did not match current orders for two medications resulting in a resident receiving 3 times the amount ordered for one medication and 4 times the amount ordered for the other medication. |
