A resident was improperly discharged on 8-17-08. |
The resident was discharged 8-17-08 after having been given a 30 Day Notice to Vacate on 8-12-08.The resident's family was called to pick up the resident when the facility determined his exiting seeking behavior was too problematic for the facility to manage.
There was no documentation in the resident's chart of interventions attempted to address the exiting behavior. |

Caller alleged that a resident with dementia was left alone and locked in his room
on 5-21-09. A citizen in the area heard the resident yelling inside the house and called the police. The caller indicated that the resident had been left alone for a significant amount of time.

DSS conducted several interviews that revealed that a resident with dementia was left alone in the home for approxiamately 35-45 minutes |
