Personal Care |
Caller reported that a resident’s room smells of urine. The caller reported that on 11-30-08. “stool was found all over the room, bathroom, walls wheelchair and the resident. The caller stated that staff had to be located to help clean the resident and the room. The caller reported that the bedside commode had not been changed. The caller stated that it appeared that no one had been there in awhile as Depends (adult briefs) were all over the floor, in the trash can, in the bathroom, in the closet and the drawers. The caller indicated great concern with the state of the resident’s personal care as the resident pays extra for grooming, toileting, dressing etc. |
DSS noted that the smell of urine from this resident’s room could be detected outside of the resident’s room and into the hallway. Further observations of the resident’s room revealed an adult brief inside the resident’s bedside commode. The strong urine smell appeared to be emanating from the commode and the adult brief inside. When asked about the provision of personal care services, Resident-1 indicated that “it could be better”. The resident was unable to elaborate, but did comment that the Carolina House staff “talk rough”. |

Personal Care |
Caller reported that a resident is visited frequently in the mornings between 7:30am and 9:00am. At each visit the caller reported that the resident has wet under garments and both the resident and the bed is soiled. The caller alleges that the Carolina House staff does not provide adequate personal care. The caller also alleges that the resident is not taken to the bathroom on a regular basis. |
During interviews, 6 of 8 residents indicated that were unsatisfied with how Carolina House staff provide personal care. DSS noted that each of these residents pay an additional fee for additional personal care services. DSS noted that the common consensus among these residents was that the staff provided inconsistent and sporadic care that was not indicative of what they were paying for. |

Residents Rights |
Caller stated that resident's request for assistance have gone unanswered. The caller stated that resident's have made requests only to have staff ignore their request or take an inordinate amount of time. |
3 of 6 residents stated during interviews that they had experienced or witness times in which requests have gone ignored or took an unreasonable amount of time. |

Food Service |
Caller stated that the home does not regulary serve green vegetables. The caller was also concerned that residents are not receiving their diets as directed by doctors orders. |
During interviews 5 of 8 residents indicated that they felt that not enough green vegetables were being served. DSS noted also that 4 of 7 residents did not receive their diets as listed by the home's dietary menu for therapeutic diets. |

Facility Lighting |
Caller reported that the facility is too dark. The caller reported that several bulbs are dark or poorly lit. |
DSS noted poorly lit and non functioning light bulbs on the 1st, 2nd and 3rd floors. 3 of 7 residents also stated during interviews that the facility was too dark. |

Housekeeping |
Caller alleged continual concerns with houskeeping. The caller alleged that several times requests for room cleaning by residents and staff are not honored. |
On 10-22-08 at 3:39pm, Durham County DSS conducted a series of observations in resident rooms. DSS noted that number of areas were in need of cleaning. DSS noted problems with individual resident rooms, and common areas. DSS noted problems with 7 of 8 resident rooms that were observed. DSS noted that 5 of 6 residents interviewed indicated the facility has problems with housekeeping. |

Medication Administration |
Medications were administered late in May 2008 because there were not enough staff to administer medications within acceptable time frames. |
4 of 12 sampled shifts in May 2008 were found to have fewer than 3 Medication Technicians to administer medications on 1st and 2nd shifts. Effective June 1,2008 a new staffing method was implemented by the facility to assure adequate staffing. |
