Caller alleged that there is not enough staff to provide basic minimal services to residents. The caller stated that call bells go unanswered in excess of 30-45 minutes. |
3 of 7 residents indicated that they had seen or experienced call bells going unanswered or response times to call bells that were unusually long. A review of timecard reports indicated that 3 of 5 dates sampled showed staff shortages.

Caller stated that she has been a resident of the facility for over 2 years. Caller stated the facility promised to ensure the carpet was cleaned as needed. The caller stated that the carpet has been cleaned once in the past year. The caller stated that the carpet is frequently soiled with urine and is nasty and slick due to the resident being incontinent. The caller indicated that the floor is so slick that paper towels have to be placed so that the resident’s feet will not slip while transferring. |
On 1-22-08, at 2:00pm, the AHS observed several rooms on the B-hall of Wynwood. The AHS noted that 1 of 6 residents rooms had carpet that was in significant need of cleaning and or replacing. |

Caller reported that a on 12-19-07 a resident's doctor discontinued the use of the Coumadin. The caller alleged that the Coumadin was given anyway. The caller stated further that on 12-21-07, The caller stated that after labs were ordered, a doctor determined that the level of Coumadin was elevated. |
1 of 6 records reviewed revealed that a resident received medications after the resident's doctor ordered the medication to be suspended. In addition, the facility failed to verfiy the resident's medications at the time of admittance to ensure that the correct dosage of Coumadin was administered. Medical treatments had to be administered to bring the resident's elavated Coumadin level down. |

The facility failed to ensure medications are administered to residents within one hour before or one hour after the scheduled time.On 11-19-08 during a routine monitoring visit all 8:00a.m. medicatitons had not been administered by 9:30a.m. on first floor and all medications had not been administered by 9:45 on second floor. |

On 9-30-08 the facility failed to have locked areas for the storage of cleaning agents and other substances which, if ingested , inhaled or handled could have been hazardous.Second floor laundry rooms were unlocked which allowed access to an opened container of floor cleaner, laundry detergent,spray paint,tile cleaning solution, carpet cleaner, and appliance epoxy.Furnace access doors were not locked allowing access to electrical wiring. |
